Reporting
Currently, there is no need to sign any documents to achieve full field version of kit, but a photo report is required. Photo-reports are important to support the project and ensure that we can continue to manufacture and supply kits for the needs of our medical services. Please take this request responsibly.
- 1
Receipt report
A photo immediately after receiving the shipment – so that you can see you with the box and the emergency sticker on it.
- 2
Placement report
A photo in a working environment, so that we may see the kits themselves and the surroundings of the location (stabilization center, medevac vehicle, etc.).
- 3
Usage report
A photo during use on a patient or during training (depending on which type of kit you order).
Please, cover the faces of the participants, if necessary, in any convenient way.
Terms
- When we work with a requester for the first time, we send no more than three kits until we receive the usage report, and then the rest of the requested justified quantity.
- For simplified training versions of kit we will expect among the above to receive official written request and signed invoice upon receiving, as training version assembled by the sponsors support and requires official reporting.
- The project is not commercial, so you cannot purchase the kits. FARMA currently does not supply kits for civilian and/or commercial training programs.
